Requesting your medical records

Your medical records are yours. Whether you need them for follow-up care at home, for your insurer, or for your own peace of mind, we help you obtain a copy securely and in a format you can use — with translation support where you need it.

What you can request

You can ask for specific documents or your full medical file — whatever your doctors at home need.

Discharge summary

A summary of your stay, diagnosis and the care you received — what a doctor at home needs first.

Consultation & test results

Specialist letters, laboratory results and pathology reports relevant to your care.

Imaging

Your scans (MRI, CT, X-ray, PET) on disc or in digital form, with the radiology reports.

Operative & procedure notes

Details of any surgery or procedure, for continuity of care with your own doctors.

How to request them

Step 1

Ask your coordinator

Contact your international patient coordinator or our team and tell us which records you need and why.

Step 2

Confirm your identity

To protect your privacy we verify who you are. An authorised representative can request on your behalf with your written consent.

Step 3

Receive them securely

Your records are shared with you securely — digitally or as otherwise agreed — and never released to anyone you have not authorised.

Format, language & privacy

Records are usually shared digitally, with physical copies or imaging on disc where you need them. If your doctors at home work in another language, your coordinator can help arrange a translation so your records are genuinely usable when you get there.

Your information is confidential. Records are released only to you or to a representative you have authorised in writing, and are handled in line with applicable data-protection law.
